Metformin for Longevity: What Is and Is Not Proven

Metformin and longevity: observational survival data in diabetes, no completed trial of aging outcomes, and why it is a prescription drug for a doctor.

Quick verdict

Metformin is a prescription diabetes drug. An observational UK study found people with type 2 diabetes who started it lived at least as long as matched people without diabetes, but that cannot show metformin caused it. We found no completed randomised trial of metformin for aging outcomes in healthy people. This page gives no dosing.

The short answer: metformin is a prescription diabetes drug, and its longevity reputation rests on observational data. In UK primary-care records, people with type 2 diabetes who started metformin had survival that matched or exceeded matched people without diabetes. That cannot show metformin caused it, and it does not apply to healthy people. We found no completed randomised trial of aging outcomes. Talk to your doctor; this page gives no dosing or sourcing.

What the evidence shows

Grade: Weak for longevity in people without diabetes.

Bannister 201478,241 people on metformin monotherapy, 12,222 on sulphonylurea and 90,463 matched controls without diabetes; adjusted median survival in controls was 15% lower than in the metformin group (survival time ratio 0.85, CI 0.81 to 0.90)Observational; who receives metformin is not random; people with diabetes only
Aging trialsWe found no completed randomised trial of metformin for aging outcomes in healthy peopleThe TAME trial is designed to test this but had not reported in what we read

The boundary

Metformin is prescribed and monitored, for example for kidney function. Whether it makes sense for someone without diabetes is a medical decision. See berberine vs metformin and berberine for the supplement comparison, rapamycin for the other drug in longevity discussions, and the graded supplement ladder. For tracking glucose see Stelo vs Lingo.

Method note

Read for this page: Mortality in people with type 2 diabetes on metformin or sulphonylurea vs matched controls (Diabetes Obes Metab 2014); Efficacy of berberine in type 2 diabetes, pilot with a metformin arm (Metabolism 2008). Common questions

Does metformin extend life in healthy people?

Not shown. The best-known result is observational: in UK primary-care records, adjusted median survival in matched people without diabetes was 15% lower than in people with type 2 diabetes initiated on metformin (survival time ratio 0.85). People were not randomised, so the result cannot establish that metformin extends life, and it does not apply to people without diabetes.

Can I take metformin without diabetes?

Only a doctor can prescribe it, and using it off-label for aging is not an established indication. Ask a doctor about your own risks and monitoring. This site does not explain how to obtain or dose it.
